Birthday Party Reservation Form

Payments: All checks must be made out to The Courthouse Gymnastics. If you pay with cash make sure you get a receipt.

Parties are never guaranteed to be "private" parties. There are usually private lessons and possibly other parties going on. We feel our facility is large enough for this.

Children under the age of 2 1/2 are not allowed to participate in a party. Any party that includes children 2 and 3 years of age may only have 10 participants in the party no matter the age of the birthday child. We want your party to be safe.

All cancellations must be made 48 hours in advance. Anyone that cancels less than 48 hours in advance will lose their $50 deposit.

The host needs to call the office the week of the party to verify party time and give an approximate number of children attending. We need an approximate number to provide the proper number of staff members.

All children must wait until the instructor calls them into the gym area.

CHILDREN MUST STAY WITH THE PARTY GROUP. THEY MAY NOT GO AROUND THE GYM WITHOUT A COURTHOUSE GYMNASTICS STAFF MEMBER.

SIBLINGS OF PARTY PARTICIPANTS MAY NOT ENTER THE GYM AREA UNLESS THEY JOIN THE PARTY GROUP.

The host is welcome to go around with the group to take pictures or video. Visiting parents are not allowed in the gym area. They must stay upstairs.

Children are not allowed back in the gym area once the gymnastics portion of the party is over.
